Visibility was excellent (daylight), so that leaves equipment failure or pilot error. I read one witness report - a passenger who has landed at SF many times before. That the plane came in too low, too soon. I've also read a report that SF's automatic warning system for approach angles was offline.

Assuming all data is correct, this suggests pilot error. Could be cockpit instrument error, but the pilots should compensate based on eyeball data. Could they have been landing with the sun in their eyes?

Edit: crash was around 11:30 AM local time, so no sun in eyes.

Kind of sad it was a 777. Apparently, it was the first deaths ever on a 777. The 777 was the very first airplane I flew in back in 2007, I believe.

And I also flew Asiana airlines before. Great airlines with good safety record. Only problem I had with the airlines is that they had no entertainment console for short international flights for coach travelers.

We should wait a little with drawing conclusions until we know more. So far, we can only exclude a few of the obvious things. Seems like the wet dream of an aircraft accident investigation team, they have access to everything, plane, passengers, pilots, black boxes and at least one video.
I don't suffer from insanity, I enjoy every minute of it.

Amazing that the fatalities are so low. Still, sad to read that one of the deaths might have occurred on the ground by being run over by rescue personnel.

Looking at that from a person who's only experience is with flight sims, my question is did the pilot punch the throttle? Doesn't look like it.

I may phrase this poorly, or maybe the entire sentiment is retarded, but that can probably be attributed to two things – survivability of the scenario (landing versus take-off or in flight) and general improvement in safety and tech.

Just have a look at the video. I'm also amazed that despite cartwheeling the plane, only three people have died (two at the scene, another girl in the hospital).

Just my 2 cents:

First, the 777. The mains hit the Seawall slamming the forward fuselage onto the runway. The tail then hits the Seawall from the 20+ degrees of nose up and the engines only up to 50%. The aft fuselage rebounds back into the air, slamming down after. The aircraft attempts to cartwheel up on the left wing, but does not have the inertia, and slams down on the main fuselage again. The main fuselage remains in one piece, and the tip of the Left Wing is bent. The cabin Crew were heroic in their efforts to evacuate the intact fuselage. The structural design of the 777 speaks for itself, IMO. You would also be had pressed to find a 777 pilot that would disagree.

Flight Crew:
Came in high on the glide-slope during the VFR approach. Normal decent is 800-900 fpm. They were high, so started the decent at 1,500 fpm. Trainee in the (L) seat, 1st flight, flight Instructor in the (R) seat. VFR approach. No ILS, but the PAPI lights were functional. At 1,500' the PIC disconnected the AP including the AT. While the AT was still armed, it was de-selected.
They were then low and slow on final. They did not even realize the engines were at idle until 7 seconds to impact, even seeing 3 red PAPI Lights. At 1.5 seconds TOGA power, nose up 20+, impact.

Crew: The pilot error in this crash IMO, falls on the Capt. in the (R) seat. He was the instructor pilot. Now I don't mean to sound stereotypic, but there is a well established culture with Asian Pilots, especially Korean. IMO, the instructor was waiting for the (L) seat to correct without having to tell him he was flying an incorrect approach. It would cause a loss of Honor. Say what you want, but it has happened before. Asiana also has a checkered history of pilot training issues.

Asiana is suing both the TV Station and the NTSB. Better chance with the TV station IMO, as the NTSB has denied releasing any information at that time. Asiana identified the Flight Crew. Not the NTSB.

Yeah... maybe the mother****ers who just crashed a plane shouldn't go around suing people.

The irony is their lead attorney's name is Mee Soo Yu.

When did the NTSB deny releasing the names? From what I've heard some summer intern at NTSB confirmed these were the names. He wasn't authorized to do what he did, but this is still a failure at the NTSB.

They were terminated, and not authorized to release anything. The NTSB apologized directly to Asiana's CEO. The TV Station simply said, "sorry" on their newscast. It makes a difference in Asia. With Asian carriers it's more about loss of honor than loss of PAX (full seats, not deaths). They were humiliated on global news feeds. The crash was enough without the typical American Racial Piling On.
