Radeon 7000
Radeon 8500
Radeon 9000
Radeon 9800
Not sure about the original Radeon card, but everything after should drive dual outputs. Be sure to check the output ports - most of these have a digital/analog port (usually DVI) plus a VGA port.

Yes, though your choices would be the Radeon 7000 or 9000 PCI or original Radeon PCI. There were some earlier PCI cards, but they have minimal support under OSX. You wouldn't be very happy with them - the interface on connected monitors would constantly be lagging.
On the nVidia side, all the cards from the GeForce 3 onwards support dual monitors out of the box. Note that the cards have to be Mac Edition ones, with Mac firmware. Many people have had success flashing the firmware of PC versions to Mac compatibility, but there have been plenty of failed flashes too.
The Mac OS supports as many monitors as you can plug graphics cards in for. Using dual head graphics cards, you could reach 8 monitors if you filled all your slots. AGP + 3 PCI.
I zapped your other thread since we're covering the answer here.
