I don't know why, but there are two extra copies of "Login Items" in the "Other" category of my System Preferences. My ~/Library/PreferencePanes/ is empty, and I only have a couple of third party panes in /Library/PreferencePanes/. I certainly don't have extra copies of Login Items around. (Just checked with Find File.)

I'm running 10.2.1.

I was going to log in as a different user to see if the problem persisted when I noticed that my "Accounts" preference pane had disappeared entirely!

Then I found a ~/Library/Caches/com.apple.preferencepanes.cache file, and I deleted it, and my problems all went away.

So.. problem solved.
