when a user is banned by request does this mean that the banned user asked to be banned or that another member asked for them to be banned?

It would have to be the first one, someone asking for a timeout. A few people have asked for temp-bans when they needed to concentrate on work or school for a time. A few others have asked for themselves to be permabanned, for reasons I don't fully understand.
We generally don't ban a member on another member's request, even if you ask very nicely. Except for spammers of course. But those aren't real members, so they shouldn't count.

It's not like the can't cave and re-register; but maybe the effort of doing so is a sufficient disincentive.

Ok, so the next logical question is that if someone requested and received a perma-ban decides to want in again. Would that be possible? or is he required to re-register?

AFAIK that has never happened. If it were to happen, we wouldn't mind if he registers again and starts posting again. I don't think the admins would mind unbanning the old account. (Obviously, this is only relevant for permabans on request, tempbans-on-request naturally expire.)

Maybe I'm missing something here but if someone needs a self-admitted timeout, can't they just not navigate to the forums? They need a temp-ban to concentrate on work of school? Maybe they need a temp-ban from the internet.

Yeah, I would have thought that just not clicking their bookmark/typing "forums.macnn.com" into the address bar would have done the trick. Personally, I enjoy posting here, but it's not sufficiently enjoyable that I can't stop myself from doing it. Checking my email, however, is a different story...
