Quite an unrealistic point of view. Context is everything. To be specific, it is used as an insult against somebody in one case, whereas it's used in reporting on that incident in another. There's an enormous difference between a bunch of people shouting "Faggot!" at a kid and him crying to his mother that he got called a faggot.
Moreover, saying she was just joking is the sort of excuse that little kids pull out when they're being scolded by their parents. Yes, it occurred in the middle of a joke, but there's no indication it was itself a joke. She was insulting him, plain and simple.
