That really is annoying, and it looks a LOT like it's one of those "your computer is compromised" scams, or one where they are trying to collect your forum login credentials. Either way this is bad. I'll pass it on to the business folks.

The kind where you can specify what kinds of ads you want or don't want and are then GUARANTEED that you NEVER get the kind of ads you don't want?

Generally, our ad services are good. But they themselves are subject to having an advertiser pull a fast one, or to the advertiser being hacked. Nobody can guarantee "never" getting an unwanted ad, but most (including ours) will pull ads that don't fit the setting if they slip through. Ours does it pretty quickly.

BTW, it wasn't a pop-up asking me to download anything - it seemed more like an automated loading script that attempted to access a file in a password-protected area of the site.

Aside from the advertiser being hacked, one would think the whole "advertiser pulling a fast one" thing could be thwarted by having humans review each ad before putting it live on the server.

Does the ad service actually host the banner, or does it just link to them? I thought it was the latter, in which case human review is easily thwarted.

It's all links, and thus it needs human supervision. Advertisers are often aggregators of banners, and so it's pretty easy to "pull a fast one" at some level several steps away from the target pages.
