Specifically Time Machine. I've replaced the .icns in the preference pane package and the Finder adds it to the package icon, but it still shows the original inside System Preferences' pane grid.

As it turns out, the old icon must have been stuck in System Preferences cache, because this morning my glorious C&H Time Machine icon is part of the grid.
