Logo Creation Questionaire?

Just curious if anyone uses a questionnaire to give to their clients to help them create a logo that they are looking for?
I know sometimes artist hold these types of things to them selves, especially if they created it, but was hoping that someone would share... i am just starting with some logo design and would think this would help

no, i do not use a questionaire.
i meet with my clients and discuss their business model. in the course of that conversation i'm generally able to ferret out the info i need to create a strong, unique id suitable for their needs. this helps me because ever client and every project are different. no template of questions can uncover the beauty of those meetings.

ask what they don't like; color, shapes, other peoples logos...

You know they have books out there that have examples of these questions and forms. They are readily available at your closest Barnes & Nobles store.

What art director said. I saw one of these questionnaires once though, I laughed my ever loving ass off.

I ended up just sitting down and writing my own, took some time but its all done.
