I was just over at Belkin's site, checking out an iPod Touch case when I noticed a couple of them sporting a small 5mm dia hole on the upper left side.
What's that about???

On the back or the front? The light sensor is on the upper left of the front, so it has to be uncovered.

the iphone and the ipod touch are basically identical shapes, I would imagine it's for the iphones speaker...

The iPhone and iPod Touch are not the same shape - I believe the iPhone is a bit taller and is definitely thicker. I don't think cases are very interchangeable.

Errr... not really. The screen size dictates the overall shape, but the differences are

-iPhone is thicker and longer (already said)*
-size of the screen glass*
-size of the home button
-iPhone has an earpiece speaker at the top*
-iPhone has a proximity sensor at top*
-sleep/wake button position is opposite on iPhone
-iPhone has headphone jack on top*
-iPhone has a camera on the back
-iPhone has a silent switch and volume controls on the side
-iPhone has the speakerphone speaker and microphone on the bottom

*Asterisk indicates where iPhone 3G's design differs from the original iPhone, which is why the 3G also requires all-new cases. All those things are places just subtly differently enough to make the original cases not work right.
