This really bugs me. I do not even listen to music but I have very long audiobooks. I have a very simple smart playlist:

1. Name is "blah"
2. Play count is 0

This used to work. When I was done listening to a section, it would hide it from the playlist. Now what it has been doing for the past few revisions is when I sync my iPhone it puts the different book segments out of order. It is a real pain when you do not notice it right away. Some of my books are travel books so it is possible to listen to them out of order for a while before realizing it.

Here is hoping iOS 4 fixes it.

I'm also pretty annoyed that it wasn't fixed in iOS4, especially after "nested playlists" was listed as a new feature.

Here's how I work around the problem:

Create the smart playlist as normal in iTunes (mine is a mixture of all the podcasts I haven't listened to and 8 hours' worth of 3-star-or-higher music I haven't heard in the last 8 weeks).

Use the far-left column as the "sorting" column (I think it's supposed to signify manual sorting)

Click the shuffle button to randomise the playlist.

Sync the playlist.

Now listen to the playlist with shuffle turned off, and you will get the songs selected by iTunes played in a random order. If you watch a video or play another playlist, afterwards you just open up the playlist again, find the place where you were and start listening again.

So not ideal, but works well enough for me.
