Bob Dole's right hand is non functional after an injury incurred during WWII He has a pen placed there to prevent people from trying to shake his right hand. He also extends the left hand when protocol calls for a handshake.

A guy I know is missing all four fingers on his right hand. The first time I shook it it threw me off but I've come to expect it now.

In my opinion, he should really take it upon himself to offer his left hand instead.

I broke my right hand last year and always offered my left in it's place when it came time to shake hands with someone. Some people would still use their right, others used their left. As long as you make the effort, it doesn't matter much.

You are hardly the first person the amputee has encountered that they want to shake hands with. Stick out your hand and let them take it from there. It's not going to be embarrassing to the amputee, so don't be embarrassed yourself.

Offer your right hand, aim for the other guy's middle. Most amputees I've worked with have gone with a left hand shake because plenty of other people are creeped out by prostheses.

Side note: just about the very first thing everyone who gets an "iLimb" hand prosthesis does is rotate the wrist 360 degrees. In both directions. It's rather odd to see the first time, but it's an interesting training task for the wearer.
